SERVICE AGREEMENTS

Requirements

AMUD requires a signed Service Agreement, Service Application and meter deposit be placed on all metered water locations with in the AMUD District.  Fill out the appropriate Service Agreement and mail or fax back to the office.

                    In District Service Agreement and Customer Information Packet

                    Non District Service Agreement and Customer Information Packet

We will start service as soon as we receive:   

• Completed Service Application and Service Agreement

• Copy of current Drivers License or Picture ID (Picure ID must be clear and readable)

• Meter Deposit, Account and Maintenance Fees

Water service can begin on the same day if we receive information and fees before 2:30pm

Please allow 2 weeks for water/sewer taps to be installed.

For your security we will only release personal account information to those people identified on the Service Agreement.
